OUR STEP-BY-STEP GUIDE TO STARTING SERVICES WITH MN CARE ACCESS

We receive a referral from a case manager, family member, or individual and promptly reach out to introduce our services and gather basic information.

We conduct a detailed intake to understand the individual’s needs, goals, preferences, and current support plan.

We collaborate with the individual and their support team to align services with their Coordinated Service and Support Plan and finalize service details.
